Before the first practice
Three forms clear every athlete.
The FHSAA requires all three on file before a student may practice or compete — in every sport, every school year. Start here, then check the sections below for anything specific to your student.

Preparticipation Physical Evaluation
Completed and signed by a licensed physician. Bring the finished form to the Athletic Office.

Consent and Release from Liability
Signed by both the student-athlete and a parent or guardian.

ECG Screening Election
Records your family’s decision on electrocardiogram screening as part of the physical.
